What is a Health Home?
A Health Home provides care coordination through a network of care management providers who work together to coordinate care for Medicaid members with complex healthcare needs such as mental health issues, substance use problems, HIV/AIDS and/or multiple chronic conditions.
Care coordination services assist clients with accessing community-based services that address their physical, social, and emotional needs.
Care Coordinators help individuals and their families understand the importance of accessing services to address behavioral health, medical conditions and other social determinants of health, while ensuring that all treatment team partners are working together.
