Immigration and Refugee Medicine

Whitney Young would like to help you complete the Immigration or Refugee physical process as easily as possible.

Your physical and follow-up consist of three visits, generally over a three week period. These visits are necessary to complete your health assessment and physical, to read the tuberculosis test and to follow-up on laboratory tests and immunizations.

In planning for your first appointment, please plan on bringing the following items with you:

  • Passport
  • Insurance Information
  • Immigration Physical Form (for Immigrants)
  • Medical Examination of Applicants for United States Visas form (DS-2053 or OF-157) and chest x-ray reports if available (for Refugees)
  • Immunizations Records
  • Insurance/Medicaid card if you have it
  • List of medications or the bottles of medications you are taking including prescriptions and over-the-counter medicines (include vitamins, birth control, herbal supplements)
  • Name, address and telephone number of your Primary Care Provider (Medical Doctor or Nurse Practitioner) if you have one

To schedule a physical, please contact Whitney Young at 518-465-4771, ext. 3506.
