2. Choose a provider

3. Complete any forms that may be mailed to you prior to your first appointment. 

4. In addition, when visiting any Whitney Young program for the first time it is important that we have your accurate medical history.  Please click here to print out a RELEASE FORM that you will need to complete so that your medical records may be sent to us. Please call us in advance at 518-465-4771 to get the proper mailing address of your new provider since the mailing address for all Whitney Young locations/programs is not the same.  Or you can request the medical records directly from your previous doctor or hospital. Some offices only keep medical records for a certain amount of time as required by the state, so call ahead to your old doctor's office to make sure the office still has your records on file. 

5. Please be sure to bring all medications you are taking and your insurance card with you to your appointment.

You may call us any time during our regular business hours to schedule an appointment.

If you are not able to make your scheduled appointment, please call the appropriate office to let them know you need to cancel. At the time of your call, you may reschedule your appointment.
