If you have a concern about your care as a MMTP patient, please report it immediately to your MMTP provider, a supervisor or to the program director. They may be contacted at 518-463-3882.

If concerns related to your care at MMTP cannot be resolved by this health care organization, The Joint Commission may be contacted at 800-994-6610.

The Joint Commission addresses all complaints that relate to quality of care issues within the scope of their standards. These include issues such as patient rights, care of patients, safety, infection control, medication use and security.
